For a player to win a Premier League medal, they must have made at least 10 appearances in the title-winning season up until 2011-12. Since the 2012–13 season, a player needs to have played in a minimum of five matches for a title-winning team to qualify for a medal.
Or, at the discretion of the Premier League board, additional medals can be awarded to players who played less than five matches. In this instance the player has been marked with an (*).
Players with 'de', 'der' 'van', or 'van der', have been allocated to the relevant letter of their last name.
For instance, Marco van Basten would be considered as in the V section. You wouldn't say Basten was a great player, but van Basten was a great player. Fight me.
