Seems out of place that John Quincy Adams is listed by his middle name, but everyone else by their last.

Took it again. Got 'em all, said I got 41 of 43. Also, you might put "Bill/William" and "Jimmy/James" for Clinton and Carter.

There's a few ambiguity problems. James could be used for Carter. William could be used for Clinton. Thomas could be used for Wilson.

A typo; Rutherford, not Rutheford.

I seem to have picked the wrong George for Washington, the wrong Franklin for Roosevelt, the wrong James for Madison and the wrong Andrew for Johnson.

I tried this quiz twice. I got them all correct both times, as others have said, but I scored a 39/43 both times as well. What is the issue? Where is the inconsistency?

It is because of the presidents that have the same names. One George is supposed to go with Washington and another is supposed to go with Bush. If you mix those up, it says you got them wrong. That's just my guess.
