Sudbury, ON is far enough from Toronto and big enough (157,857 population in the 2006 census) to make the list. Similarly, Saguenay, QC is also far enough from Montreal and big enough (143,692) to make it. Kelowna, BC (population 106,707) is also big enough and far enough from Vancouver.

By a great circle measurement, Honolulu is closer to San Francisco than San Diego.

I've added Sudbury and Saguenay, and changed the figure for Honolulu. Kelowna, on the other hand, is only 170 miles from Vancouver.

At current rates of population growth, Las Cruces is probably over 100,000 by now. Maybe it should be here.

I didn't add it because there isn't a formal number for it that's over 100,000. I'll add it as a bonus answer.

And if this were by road, Honolulu would be an awfully hard to figure out.

For the purposes of this quiz, should the CFL count as a major sports league?

I think if the CFL were counted, only Regina and Saskatoon would be removed (due to the Saskatchewan Roughriders). Go Riders!

Using Google Earth, it looks like Fargo is slightly closer to Winnipeg (about 208 miles) than it is to Minneapolis.
