Nice idea, but it might be better if you had to select the team. Also: Oakland Raiders (LA), Oklahoma City Thunder (Seattle), New Orleans Hornets (Charlotte), LA Lakers (Minneapolis), LA Clippers (Buffalo), Atlanta Braves (Milwaukee, before that Boston) and a bunch of other teams would fit this

Bad execution. Dynamo and Ravens never officially relocated and were considered new franchises. Washington needs to be accepted. The North Stars played in Bloomington, not Minneapolis. Quizzes like this should force you to answer each individual question.

You could have added another 20 or so moves to this list. Avalanche's prior locale should be Quebec City.

Also, the Rams actually started out in Cleveland, so be careful about including teams that have moved more than once.
