	Tough quiz. I suggest you reword question 7--first, you need to establish whether the musical period in question is the early, middle, or late medieval period (important because polyphony didn't begin to be codified until nearly 1000CE). Second, chords were not "banned"; there was no watchdog group telling musicians what they could and could not do, merely the parameters of established practice. Music at the time was built on 3 hexachords (six-note scalar groups with consistent intervallic relationships), with B natural (known at the time as H) introduced in the hard hexachord (B referred to what we now call B flat, introduced in the soft hexachord). Because F sharp did not exist (as well as E flat; note that there are actually three correct possible answers in the options you give) it was impossible to form a perfect fifth relationship with H (B natural)--attempting to do so when singing polyphonically would produce a diminished fifth, or tritone, which medieval musicians were taught to studiously eschew as it was believed to be disruptive. To avoid this, at first singers would simply lower the H to B (flat), thus producing a perfect fifth; later, someone had the bright idea of sharping F to solve the problem another way. Usually this was not notated in written music and left to the judgment of the performer to correct. Sorry for the lengthy response; I teach music history at the collegiate level and couldn't let this slide by.
