| I think spironolactone, triamterene and amiloride should be grouped together as one answer: potassium-sparing diuretics. You can then make the three individual drugs acceptable alternative answers, along with "diuretics." I know most diuretics are not K-sparing, but anyone playing this quiz is going to enter "diuretic" with the knowledge that he's referring to the K-sparing group. Also, in addition to rhambdomyolysis, I'd include the more general terms "injury" or "trauma." Anything that causes massive tissue damage, including crush injuries, severe burns and electrocution will lead to cell membrane disruption and intravascular spillage of potassium. Lastly, I think DKA should be grouped with the more general "diabetes." A patient with poorly controlled IDDM will have low insulin levels resulting in hyperkalemia. It's not particularly uncommon to find hyperkalemia in newly diagnosed IDDM, even without DKA. |
