Highest-billed refers to the two people whose names are uppermost on the film's theatrical poster or leftmost in some cases. If only one name is present, screen time will determine the second actor.

How Peter O'Toole isnt there for Lawrence of Arabia or why Alec Guinness isnt there for Bridge on the River Kwai is beyond me

I was equally surprised. But if you look at the 'Lawrence of Arabia' poster, O'Toole is credited last. And I'm having trouble even recalling Jack Hawkins' role in 'Bridge.'

I was also shocked to not see Vivien Leigh be in the top two billed for 'Gone with the Wind.'

"Lawrence" was O'Toole's first movie so thats probably why, though i agree he should've been up at least no. 2

Your sources may be suspect. Most every lobby card and one sheet from Bridge on the River Kwai's original run has the top billing as Holden, Guinness, and then Hawkins. The "and presenting Vivien Leigh" makes sense for Gone with the Wind's original run but I'm sure a few of these aren't totally accurate.
