If Birmingham and Tuscaloosa are considered Appalachian than Chattanooga, TN has to be...if so it would be 5th on your list. Also, Asheville, NC is 100% Appalachian and it would be about 7th or 8th.

Check the U.S. government's map of Appalachia, from the Appalachian Regional Commission at http://www.arc.gov/appalachian_region/MapofAppalachia.asp. It appears that both Chattanooga and Asheville are in the "official" zone.

According the ARC map, Winston-Salem, NC is also an "Appalachian" city
