I thought that a lot of people in South africa and surrounding nations also spoke english

I originally thought that was the case but in South Africa it turns out there are more than 10 primary official languages.

But the quiz doesn't say 'countries where english is the official language', it says countries where english is the most spoken language. Which is it meant to be? This could be good if you can clarify this.

Only something like 8% of South Africans speak English at home/as their main language. Xhosa, Zulu, and Afrikaans are far more common.
