Before anyone asks where Voldemort was ever called "Chief Death Eater", it was in the Deathly Hallows on the radio show "Potterwatch" run by Lee Jordan.

Thanks for the suggestions, Voldy is now an answer and the others are bonus answers. And who voted down my first comment? I was just explaining the title "Chief Death Eater" because it's only used once in the series. I thought someone might wonder where that name came from.

Wasn't he also called "Lord Thingy" by Fudge, in a quotation in the newspaper?

u-no-poo wasn't a name for voldemort, it was just a play on you-know-who, reffering the the constipation sensation that;s sweeping the nation.

Just wondering, should I include Voldy and Lord Thingy as answers or bonus answers? People seemed to want them, but when I included them, the ratings went down.

@aysee: This was my first attempt at a colored quiz, so they may not be great, but they are readable, and I don't have a problem with them. If anyone else doesn't like them, feel free to tell me, or just aysee's comment up.
