	I would also add the Toronto St. Pats and the Vancouver Millionaires.

	I've only done a quick Wikipedia search regarding those two teams, but my understanding is that the Millionaires weren't ever actually a part of the NHL, only the PCHA and WCHA, and the St. Pats never actually went defunct or relocated, but instead were part of the series of teams that evolved into the Leafs. I'm now thinking about adding them as bonus answers, though!

	Yep - Millionaires were never an NHL team (the Cup, at first, wasn't exclusive to the NHL), and the St. Pats were just a previous name of the Maple Leafs - same as Ducks to Mighty Ducks or Cougars/Falcons to Red Wings.
