Does 42nd/Grand Central take into account the Metro-North side of the stop and does the 34th/Penn Station take into account the LIRR side of the stop?

no, the passenger counts are only for the New York City Subway and do not include Metro-North, Long Island Railroad, PATH, NJ Transit, etc.
