	Voldemort was played by Richard Bremmer in the flashback scene in Philosopher's Stone and was voiced by Ian Hart when he was shown on the back of Quirrell's head. Possibly another answer or a bonus maybe?

	No, Flitwick is played by the same actor, Warwick Davis, throughout the series. In the first two films, Chris Columbus' crew styled him as an elderly drawf-like man. However, in the PoA script, there was no room for Flitwick, so the producers asked Davis to play the choir conductor (credited simply as Wizard), styled with the black hair and moustache. For GoF, they decided that the choir conductor was actually Flitwick after all; Warwick Davis has since said he considers this new wizard to be a cousin of the original Flitwick, but officially it's the same character, played by the same actor.

	You might also accept Bill Weasley since is played by Richard Fish in the third film (in a photograph) and Domhnall Gleeson in the seventh film.
