Alternate spellings would be good, like Andijan, Turkmenabat, and Shymkent.

Karagandy, for example, not just Karaganda. Otherwise, this game is impossible, as there are too many variants on how to spell the city names in English.

Well obviously it's English spellings only, or else you'd be including answers in Cyrillic. But the point that others are making is that many of these cities do not have one universally agreed-upon spelling in English. And if you don't want a quiz based just on Pakistan, how about increasing the total number of cities? Would make a lot more sense than arbitrarily leaving it out.

I don't want to accept tons and tons of different spellings for them. Honestly, the most obvious ones (like Tashkent and Bishkek) many people will know how to spell, but the less obvious ones, so many people will google or look up and these spellings come up on sites like wikipedia and my source. If you want to play a quiz which is largest cities in Pakistan then do it.

I understand the exclusion of Pakistan, as two thirds of the quiz would be Pakistani cities and take away from learning the cities of the central -stans, which Pakistan is less related to. As for accepted spellings, when something is transliterated from a non-latin alphabet language, there are numerous possible spellings (like Gadhaffi or Khrushchev) and quizzes should accept as many as possible.

Since there have been a load more plays due to the Kyrgyzstan thing, I'm checking every city and accepting alternatives.

I would use a different source. Yours uses population estimates from 2002, which I don't think is useful for a quiz from 2012; Astana, for example, is now the second largest city in Kazakhstan. I would suggest Geohive, which has both UN estimates and country census results.
