I don't know how many answers you accepted for "varies" and "same as nominative but I always learned it as "?" Maybe you could add that or n/a, etc.

Seems fair. The only tricky part is that I'd like to make it so that the quiztaker has to show they know that the neuter accusative is same as the nominative (which is why I don't accept "varies"), but I did add "?" and "n/a" for both. I changed my other quizzes for the appropriate spots, too.
